Medical review
After your medical screening, you may be given a Physical Employment Standard (PES) D. This means more information is needed before your PES can be determined. This can happen if more relevant documents and/or medical investigations are required. In the case of the latter, you may be asked to attend specialist appointments at restructured hospitals.
The actual number of medical reviews that you will be required to attend depends on the nature and complexity of your medical conditions. These medical reviews are critical and important to allow us to determine your medical fitness for full-time National Service (NS).

Scenario 1: Need more medical documents
Medical proof or medical documents
Our Medical Officer requires more medical proof or medical history documents from you.
Send documents and attend appointment
You are to submit the required documents through OneNS by the deadline stated in the FRO and attend any follow-up appointment as required.
You may extend the submission deadline through OneNS if you are unable to obtain the required documents by the deadline. Note that you can only extend the deadline once.
Check your PES
Check your PES online one month after you have submitted the documents, if no further action is required.
Scenario 2: Appointment with the Medical Classification Centre (MCC)
MCC tests and/or reviews
You need to report to MCC for follow-up tests and/or reviews.
Attend your appointment
Attend your appointment punctually. If you need to change your appointment date, you may submit your request here. Please provide two preferred dates for your appointment and submit the documentary proof (e.g. Medical Certificate, School Letter, etc) to support your request.
Check your PES
Check your PES online after one month if no further action is required.
Scenario 3: Appointment with the National Heart Centre Singapore (NHCS)
Follow-up appointment
You need to report for a follow-up appointment with NHCS.
You can log in to OneNS to access your Further Reporting Order (FRO). Please print the FRO and referral letter from OneNS.
Appointment
Attend your appointment punctually.
Bring your documents
Bring your FRO and referral letter with you.
Change of appointment
If you need to change your appointment, please contact NHCS at safcardiaccentre@nhcs.com.sg to change your appointment. NHCS' operating hours are Monday to Friday: 8am to 5pm, closed on Saturday, Sunday and Public Holidays.

Scenario 4: Appointment with other restructured hospitals
Follow-up appointment
A follow-up appointment with other restructured hospitals is required after medical screening.
You can log in to OneNS to access your Further Reporting Order (FRO) which will include:
- Letter of Identity (LOI) to waive charges
- Progress Report on Medical Status (PR)
- Referral letter
Appointment
1. Attend your appointment punctually.

2. If you need to change your appointment, please refer to the 'Changing your medical appointment' section below for more information.
Bring your documents
3. Bring your FRO, LOI, PR and referral letter with you to the appointment. If you have rescheduled your appointment, please download the updated documents from OneNS.
Complete the PR
4. Get the attending doctor to complete the PR.
Submit your appointment card
5. Submit your appointment card and PR through OneNS. Alternatively, you can do so by email or mail.

Further appointments
neededYou can access your LOI and PR from OneNS. A new LOI and PR will also be mailed to you which you must bring on your next appointment.
Once you have completed all the required follow-up appointments and submitted the relevant medical documents, you will be able to check your definitive PES in about two months' time.
Changing your medical appointment
If you need to reschedule your medical appointment at any of the following places, please submit your request here:

- Khoo Teck Puat Hospital (for 2D-Echo)
- National University Hospital

- Singapore General Hospital
- Tan Tock Seng Hospital (Endocrinology and Renal Medicine)
Please provide two preferred dates for your appointment and submit the documentary proof (e.g. Medical Certificate, school letter, etc) to support your request. You will be issued with a new Letter of Identity (LOI) after your appointment has been rescheduled.
All other external medical appointments
Please call the hospital, national specialty centre or medical centre directly to reschedule your appointment using their respective contact details in the table below. Please also update your appointment details and download a new set of documents on OneNS after you have successfully rescheduled your appointment. Kindly bring along these documents for your appointment.

Letter of Identity (LOI)
An LOI waives consultation and investigative fees up to the point of diagnosis at restructured hospitals. This does not include the cost of treatment and medication that is dispensed subsequent to the diagnosis.
If you are graded PES D, we may require you to undergo further diagnosis at these hospitals and we will issue you the first LOI. For subsequent appointments, you may or may not be required to apply for additional LOIs. For changes in appointment date, you may also require a new LOI. This will depend on the type of appointment. Please refer to the above section on the medical review process for more information.
Should you require an LOI for your appointments, you must bring along the hardcopy LOI with you when you attend your appointment. If you fail to do so, you may not be allowed to attend your appointment or may have to bear the medical fees incurred for your diagnosis.
Submitting medical reports, memos or Progress Report on Medical Status
You can log in to OneNS and upload it under Manage Medical Matters. Alternatively, you can email it to us at contact@ns.gov.sg.
Requesting for details of your PES
Due to medical-in-confidence, we will require you to fill in the consent form before we can release the details of your PES. Please download the consent form and mail the signed form or email a scanned copy of the signed form to MCC. The details of your PES will be sent to you.
Important notes
Further Reporting Order (FRO)
An FRO will be sent to you prior to your medical appointment date. You can also log in to OneNS to access your FRO. The FRO specifies the medical appointment details, the required documents to bring and important instructions that you are to take note of. Attending a scheduled medical appointment is compulsory. Not complying with the FRO is an offence under the Enlistment Act 1970. If you are unable to attend the appointment, please reschedule and update us.
Please note that any changes in appointment dates of your medical review may result in a delay in your enlistment. You should try to schedule your follow-up appointments less than two months apart if possible.
Progress Report on Medical Status (PR)
This is a form which you must pass to the attending doctor at the hospital to complete after every appointment. It is an important document for our Medical Officer to determine your medical fitness for NS. The details in the form will also help determine if you are eligible for a new LOI for your subsequent appointments (if any). You are required to submit the PR through OneNS after every appointment that you have attended.
Keep any forms issued to you by MCC and the hardcopy PR for a minimum of six months as you may be requested to produce them if necessary. From the conclusion of your last referral (multiple referrals may be required), we will need about three months for your final PES to be determined. This includes time for the hospital to prepare your detailed medical reports and for review by our Medical Officer.
